DT2220/DT2221 Mix Design Modification Tab

Linked are updated versions of the DT2220 and DT2221 forms that include a new 1A Modification tab developed by WCPA. Please note that this is not an official WisDOT form at this time. A committee is currently reviewing and updating the DT forms, and it is anticipated that an official modification process will be established in the future.
The purpose of this tab is to document any modifications made to a concrete mix design while preserving the integrity of the original mix design documentation. It provides a single location to track approved changes, helping maintain clear and organized records throughout the life of the mix design.
The intended benefits of this process are to:
  • Preserve the original mix design documentation.
  • Maintain a complete record of all mix design modifications in one location.
  • Improve documentation consistency and traceability.
  • Expedite the submittal and approval process.
Also included is a Directions - Modification tab that outlines the intent of the modification form and provides guidance on its proper use/inputs.
Please note that multiple modifications may be documented within a single entry. However, the modification will be reviewed and approved according to the most stringent approval level required. For example, if a submission includes both a Level 1 and a Level 2 modification, the entire submittal will require Level 2 approval.
